Sponsored by the Peterson Center on Healthcare and convened by the National Academy of Medicine, this meeting will engage stakeholders in a discussion of the issues, challenges, and approaches that present the greatest opportunity creating models of care for high-need patients.

The third and final of three meetings, this workshop is aimed at advancing approaches, policies, and tools to enhance the spread of successful models of care delivery. (Information about the most recent workshop in the series is available here.) The discussion is intended to explore how we can better serve high-need patients, improve their health outcomes, and reduce costs.

Objectives of the October 21st meeting:

  1. Advance policy to support better care for high-need patients.
  2. Examine tools to improve care delivery for high-need patients.
  3. Synthesize and identify future opportunities.
