At this meeting of the Best Practices Innovation Collaborative (BPIC), participants will:

1. Review progress and next steps to advance BPIC’s collaborative work on the patient’s role on the care team.
2. Identify opportunities to increase the effectiveness of community care networks in engaging with health professionals and patients.
3. Identify next steps and future opportunities for BPIC participants.
